The document that opens every payment cycle.

The document you issue to apply for payment each cycle. If the payer misses its notice deadlines, a valid application can itself become the sum legally payable.

How it works

Fill in the yellow cells

Party details, dates, then the measured works and variations tables. You type the cumulative value per line; the template calculates this application as the movement against what was previously certified.

The summary runs itself

Gross valuation, previously certified, retention, VAT (including the domestic reverse charge wording) and CIS all calculate automatically, down to the amount applied for.

Mistakes get caught

Amber and red highlights catch the classic errors, like typing the monthly movement instead of the cumulative, and a pre-issue checklist sits below the printed area.

Why it is laid out this way

It mirrors the Payment Notice line for line, so the two documents reconcile exactly and any difference is visible at a glance. Prints as one A4 page.

An Excel template with no macros. Only the yellow cells can be edited, the Guide tab explains every step, and it prints on A4. Your contract's terms override the statutory defaults.
